Appointments can be scheduled during regular business hours which are Monday through Thursday 8am-4pm. The office is closed on Friday.

A new patient packet is sent out to patients prior to their first appointment. Please bring these completed forms to your office visit with you. Please arrive 10 to15 minutes prior to your appointment.

Please bring results of your labs as well as results of any x-rays, CAT scans or MRI’s with you to your appointment.

Please bring a list of medications you are currently taking with strength (mg) and frequency, including prescription as well as over the counter medications.


We accept and file insurance claims for most major insurance companies. If you have questions about our participation with an insurance plan, call the customer service number on the back of your insurance card.

If you have an insurance plan that requires a referral, you will need to contact your primary care physician, and have them forward a referral to our office. We may not be able to see you if a referral is not on file with our office by the scheduled appointment date.

Please bring your insurance cards and picture I.D. with you to every appointment. All co-pays and deductibles are due at the time of service. To make payments more convenient for you, we accept cash, checks, Visa, MasterCard, & Discover credit cards.

Prescription & Refills

Requests for prescriptions or refills will be answered during normal business hours in order to access your medical records and consult with the physician. Have your pharmacy fax over a prescription refill request to our office at (270)769-9020 at least 5 days before your prescription runs out.

Medical Records

If you require a copy of your medical records for any purpose, you may request them. Allow 10 business days for records to be ready. You will have to come into the office and pick your records up and sign an authorization at that time. Under Kentucky law, individuals are entitled to one free copy of their medical record. Once the first copy have been provided for free, a charge of $1.00 per page will be assessed for additional copies.

Completion of Forms

There will be a charge of $15.00 per page for completion of forms. However, we do not perform disability evaluations. To check the status of forms or medical records, you can contact our office at (270) 769-2535.

After Hours Care

If you should have a serious medical emergency after hours, you should call 911 or go to the nearest hospital's emergency room.
